Reassessment

As our residents age and their needs change, we make sure to reassess care plans as appropriate

Our team is trained and certified by the Alzheimer's Association in our Person-Centered approach to Dementia/Alzheimer's care. Dementia and Alzheimer's are progressive diseases that affect individuals differently that the normal process of aging. While most cases are linked to genetics, age tends to be the biggest risk factor from current research. Each person usually assessed as being at either an early, middle or late stage of the disease.

1.

Person-Centered care revolves around the concept that the unique needs and experience of each person is essential to providing the highest quality of care.

3.

We focus on establishing strong relationships and build a sense of trust between residents and staff. This allows for a supportive environment for residents to thrive in.

2.

Staff are encouraged to get to know each resident, their life, their habits and connect with them on a personal level.

4.

With time, there are changes in communication that are expected. Our trained staff will look for the triggers, verbal and non-verbal communication and respond with compassionate care.
